Depending on the dynamic of your family, the following documents may also be needed:

A consent letter is technically not required but! it will make travelling much smoother for your child. A notarized travel consent letter may be requested at some international boarders, and sometimes when re-entering Canada. This letter proves that your child has permission to travel abroad without parents or legal guardians joining them. 


Moreover, if your family has unique circumstances it is best to consult a family law lawyer prior to arranging travel for your child. 


Yes. One or both parents, or legal guardians who are married or in a common-law relationship and live together with the child. Both parents usually have custody rights over the child.


Additionally, the Government of Canada also recommends that you contact a lawyer about you and your children’s unique situation.


When selling a vehicle privately in Ontario, in most cases, you’re responsible for paying retail sales tax when the vehicle’s ownership is transferred. Nonetheless, there are situations where you can skip making the payment, and in doing so you need this form (1155E)


  • Family Breakdown – if a vehicle is acquired between spouses or former spouses after a breakup and as part of a settlement under the Family Law Act.
  • Transfer between Related Corporations or Corporations/Shareholders – if a vehicle is acquired between related companies and/or shareholders. 
  • Insurance, Financing, or Security Purposes – if the owner is only being registered in another person’s name for insurance, financing or security purposes.
  • Temporary Purposes – i.e. between a carrier and broker driver. 


  • the transferor’s name and city/municipality of residence
  • the transferee’s name and city/municipality of residence
  • the year/make/model of the vehicle
  • the vehicle’s VIN number
  • the date the vehicle is to be transferred, and
  • the reasons for the transfer

This form 1155E also needs to be notarized to be accepted by Service Canada 



  • a spouse;
  • parent or step-parent;
  • grandparent or step-grandparent;
  • son/daughter or step-son/daughter;
  • grandson/daughter or step-grandson/daughter;
  • granddaughter or step-granddaughter;
  • son/daughter-in-law;
  • father/mother-in-law; or
  • sibling, half siblings (siblings with a common parent) or adopted siblings (siblings with a common parent through adoption).

The retail sales tax is waived when the vehicle transfer is made between any of the family members above.


The transfer is pretty simple. In order to make the transfer smooth you'll need the following documents before going to Service Canada. Please keep in mind that only one exempt transfer of the same vehicle between family members is allowed within a 12 month period. 


  • a Sworn Statement for a Family Gift of a Used Motor Vehicle that can be downloaded here with quick seal from a Notary to authenticate your signature,
  • proof of Ontario vehicle insurance,
  • your Ontario driver’s license,
  • Safety Standards Certificate (please note that this is not required for spouse-to-spouse transfers),
  • Vehicle Ownership Permit with the completed portion on the back and,
  • a completed plate transfer declaration if you are also transferring the license plate (this is required on spouse-to-spouse transfers in order to exempt them from the Safety Standards Certificate).


 CCDC9A -2018 

  • Is used by a contractor 
  • Is a sworn statement 
  • They use this form as a condition of receiving payment for either the second and subsequent applications for progress payment or the release of holdback funds.

 CCDC9B -2018 


  • Is used by a subcontractor 
  • Is a sworn statement 
  • They use this form as a condition of receiving payment for either the second and subsequent applications for progress payment or the release of holdback funds. 


At the current moment there are four different associations that enforce the CCDC documents:


  • Association of Consulting Engineering Companies – Canada (ACEC)
  • Canadian Construction Association (CCA)
  • Construction Specifications Canada (CSC)
  • Royal Architectural Institute of Canada (Architecture Canada)


The declaration of the above noted form is needed as part of your application package. This form attests the truthfulness of all the information and documents to support your application for registration.


The CRPO notes the following: “If your work falls within the scope of the practice of psychotherapy and you are not already registered with another Ontario regulatory college whose registrants can practise psychotherapy (those that regulate psychologists, social workers, nurses, occupational therapists and physicians), you should consider becoming a registrant of CRPO.”

This form is required in Ontario from suppliers of construction materials. The suppliers need to declare and date which supplier of services or materials last supplied under a contract or subcontract. The form also states that the supplier will not supply any further services or materials under the contract or subcontract.




  1. A description of the services or materials that were supplied.
  2. The name of the payer, for example, the company or person you contracted with and supplied the materials too, as well as the date of the contract.
  3. The date of last supply of the materials.
  4. The name and address of your supplier company.


An affidavit is a sworn statement of facts that has been written down, usually in numbered sentences / paragraphs, and sworn to (swearing the truth by signing) by an individual (called the “affiant”) in front of someone who is authorized to administer oaths, such as a Notary Public or a Commissioner of Oaths. 


Is an affidavit that is tailored to prove ownership of a motor vehicle. This document is needed by Service Canada in order to register your vehicle. 


A RIN is a number given to a business or corporation by the Ontario Ministry of Transportation. It is used to help identify the business as the registered owner of a vehicle.


Is a letter which is sworn before a notary public or commissioner of oaths that is sent to the Ministry of Transportation (MTO). by a business requesting to be assigned a RIN for the purpose of registering vehicles under their name. 


When your request letter is approved and all information is verified. the MTO will issues a unique RIN to the business and send a letter containing their RIN number and barcode.

In some instances you may need to provide your Articles of Incorporation, or other legal documents pertaining to your business in order to verify it is a legal entity in Ontario.


In general you will need the following documents to be included? 


  • Business or organisation registration documents, such as articles of incorporation or a partnership agreement.
  • A copy of a business or organisation licence, if applicable.
  • Business or organisation identification documents, such as a business registration number or tax identification number.
  • Identification documents for the person authorised to sign the request letter, such as a driver’s licence or passport.
  • A detailed description of the business or organisation’s activities and how they relate to vehicle registration.
  • A completed and signed RIN Application Form (if applicable).

No, there is no fee from the Ministry of Transportation regarding getting a RIN number for your company vehicle. 


On average it takes approximately 10 business days, subject to all necessary documents being included. Another factor may include how busy the MTO is.
